Consult your Community Health Assessment and Improvement Plan (CHA/CHIP)
Your Community Health Assessment and Improvement Plan (CHA/CHIP) can help you identify priorities and opportunities for evidence-based prevention as well as community assets that can help you achieve health and wellness in your jurisdiction. When preparing to carry out evidence-based prevention initiatives, check your CHA/CHIP to identify the public health concerns, high-risk populations, epidemiologic patterns/risk factors, and the individual, social, environmental, political, and structural determinants of health and quality of life.
